What is Community Health?

USD 250 understands the role that the social emotional and physical wellbeing of an individual and family effects their education and ability to learn. Through a continued partnership with CHCSEK and the Telehealth Rocks grant, USD 250 now has a Community Health Worker to help meet the needs of our Dragons. 

The role of a Community Health Worker (CHW) in schools is to improve families' access to care and aid them in finding resources and support them in the process of scheduling appointments, transportation, delivery of prescription medications, connecting students to care and utilizing technology to better connect students to their care team. The CHW assesses the families Social Determinants of Health (SDOH) to personalize their care and to get a baseline of their needs.


 

 

Meet Kaitlynn

Kaitlynn started with CHC in July 2022. She has a degree in Family and Consumer Sciences with an emphasis in Community and Family Services from Pitt State.

Kaitlynn has lived in Pittsburg for 10 years. Her personal life experience has helped her to better understand the families she works with and provides a solid foundation of knowledge to give them a helping hand. Her favorite thing about being a CHW is building relationships with families and coming up with solutions that will meet their individual needs.
